Location: This marker was located on the southwest corner near the old west entrance to Welborn Hospital on Fourth and Oak Streets in Evansville. Arrow is pointing to the plaque. This plaque was removed from the building and placed in storage by the Southwestern Indiana Mental Health Center which purchased the building. They are willing to donate it to someone willing to publicly display it. It is hoped that it will be placed back in its original location at Fourth and Oak Streets.

Honig's name appears at the bottom right. Written on a Honig sales brochure in the Sigma Chi collection are these comments written by George. "Joe Cook Tablet erected on spot where Joe learned to do those stunts as a boy. Joe came to my studio. Happy and pleased with my work. I made him a plaster copy of the original. He lives in the state of New York."
